Clutch disk had separated from the center splines

I am having the same problem with my 94 l wagon Ifound that the clutch disk had separated from the center splines on the input shalf. I replaced the clutch flywheel throwout bearing. And sure enouyit happened again this time I only got 600 miles out of it anybody out there that can help

What brand clutch did you use, how many miles are on the car, and is there anything else you can tell us that might help in troubleshooting the problem?

Sounds like an issue with the clutch disk quality, worn input shaft bearing or lack of dowel pins at the bellhousing that align the trans to the engine.
My guess is dowel pins lol
