I would say it depends on milage on said parts. Also on the wear when you check them. I have ALWAYS replaced them all. Pilot bearing, clutch, flywheel, release bearing(throwout bearing), rear main seal. Just to be safe, it's a real bitch to get the engine tranny out to replace them so you might as well do it while it's out.

I've always bought exedy clutch kits on ebay because they come with all these parts aside from the main seal which can be bought at the dealership easily and relatively cheap ($12 probably).

Yep, that should do it.

Also make sure to check if that plastic backing plate on the back of the engine is leaking. I replaced mine with a metal one I got from the dealership but you also need different bolts because the plastic one uses screws. They're the same as the oil pan bolts.

That plastic POS is broken on my car... annoying.

I have never actually replaced a flywheel unless it was badly heat cracked or abused. Usually you can have them resurfaced (for $15) and they will be just fine. Mine's been resurfaced 3 times now, probably will replace it if I put a different gear-set in this year. Just MHO.
