92 Auto. Reverse is neutral unless the car is cold AND the weather is colder than 70. fluid is good. no noises. and if its cold and it works it only works at idle. if i give it gas it just stops and just idles until it slowly starts to creep again. i have searched and found other posts but alot were old and didnt really have much of an answer, what most likely needs to be replaced. it has 250k miles but every forward gear works PERFECTLY better than my girlfriends 40k mile ford fusion in my opinion

Have you checked the fluid to see if it is at the correct level and is "pink" in color and doesn't smell burnt? I'd start there.
If everything checks out ok, it could be clutch or something internal to the transmission. Short of tearing into the trans, it will likely be a little difficult to diagnose.

If fluid and everything is good, as I mentioned above, short of tearing into the trans, there's not going to be much you can do. Sorry.
