First of all make sure your crank pulley is not loose and messing things up. If it is loose lets just hope it has messed up your belt tension. Also make sure you are tightening both bolts on the alternator, the pivot bolt and the bolt in the adjuster. While you are at it make sure you ps pump is secure. That all I can think of.

I'll check the crank pulley,everything else has been tightened multiple times.

maybe the bolt/nut is stripped on the adjuster and coming loose?
